•Your Social security number

•Your School (eg. Arts and Science, Engineering, Graduate School, Peabody College, ...)

•New address(s):

•Permanent address and phone number
For undergraduates, this is your parents' address. For graduate and professional students, this is the address of a person who can always contact you.

•Second parent's address and phone number
(if applicable) - For students whose parents live at separate addresses

Off-campus local address and phone number
(if applicable) - Where the student lives while enrolled in classes (not VU residence halls)


•To which address should your grades be sent?

•To which address should your bills be sent?

•Phone number where you can be reached between 8:00-4:30 p.m. (central time)
