for filling out the application:
1.
Download the application form and save.
2.
Open the file in MS Word 98 or higher.
3.
Click in the gray boxes to fill in your information.
4.
When you are finished, save the file. For instance, if your
last name is "Smith," you would save your application
file as: smithIUTapplication.doc
5.
Email your application as an attachment to sandie.frantz@
vanderbilt.edu
6.
When submitting by email, set your email message to return
a read receipt so you will be sure it has been received.

You may also print out the application and hand deliver
