for filling out the application:
1.
Download the application form and save.
2.
Open the file in MS Word 98 or higher.
3.
Click in the gray boxes to fill in your information.
4.
Print the application.
5.
Submit application by campus mail, fax or in person at the 1st floor of the Baker Building.
6.
When submitting by email, set your email message to return
a read receipt so you will be sure it has been received.
